(57) A fall protection permanently embedded safety anchor comprises a unitary housing having outwardly diverging side walls and an interior cavity. Opposed apertures in the side walls receive a uniformly curved retaining bar, the ends of which extend outside the cavity and are embedded in the concrete. The anchor may be easily installed stored and transported and installed in the field. The side walls terminate in an outwardly extending collar that receives a cover that is flush with the surrounding concrete surface.
